Step 1
Visit our Frontdesk Clerk at our office located at Allen Avenue Extension, Catbalogan, Samar for New Connection.
Bring the following documents
1. Brgy Certification / Tax Declaration
2. Community Tax Certification

Fill up a New Service Connection Application Form. click here to download.
Step 4
Engineering division will conduct Investigation and Survey for cost estimates. The investigation, survey and cost estimates shall be completed within 1-2 working days. An sms will be sent to your cellular phone for the results or you will be advised to follow up after 2 days.
Step 5
Upon completion of the estimate, the customer shall be advised to pay the bill of materials and other required fees to the Cashier. after payment, frontdesk clerk will ask you for your signature on the water service contract.
New service connections will be installed within 1-2 working days from the date of payment and water contract signed.
Step 3
Attend Seminar for New Water Service Connection scheduled every saturday, 9:00 a.m. in the morning at CWD office. This seminar is a prerequisite in the application for new connection.
This seminar is conducted to discuss the content of the Water Service Contract. This is also to answer any questions related to the water connections and other services that the district could offer.
